California IHSS: In-Home Supportive Services Program Complete Guide

California's In-Home Supportive Services (IHSS) program is the largest publicly funded home care program in the United States, serving more than 700,000 Californians and paying over 600,000 caregivers. For low-income seniors, people with disabilities, and the blind, IHSS pays for personal care and household services in the home so that recipients can avoid moving to a nursing home or care facility. Importantly, IHSS allows family members (including adult children, but not spouses or parents of minor children in most cases) to be paid as the caregiver.

What Is IHSS?

IHSS is a California Medi-Cal program administered by county welfare departments and the California Department of Social Services. It is funded jointly by federal Medicaid, the State of California, and county governments. There is no waiting list. If you qualify, you receive services.

What IHSS Covers

IHSS pays for a broad menu of services, organized into categories. The number of authorized hours depends on a county social worker's assessment of need.

Personal Care Services

• Bathing, oral hygiene, and grooming.
• Dressing.
• Bowel and bladder care.
• Transferring (helping someone move from bed to chair, etc.).
• Eating and feeding assistance.
• Ambulation (helping someone walk safely).
• Repositioning and rubbing skin to prevent pressure sores.

Domestic and Related Services

• Housecleaning.
• Meal preparation and cleanup.
• Laundry.
• Grocery shopping and errands.

Other Services

• Accompaniment to medical appointments.
• Protective supervision (for people with cognitive impairment who cannot be left alone safely).
• Paramedical services (under physician orders, such as injections or wound care).

Who Qualifies for IHSS?

To qualify, the applicant must:

• Live in California.
• Be 65 or older, blind, or disabled.
• Be eligible for Medi-Cal (the California Medicaid program), which has its own income and asset rules. As of 2024, California eliminated the asset limit for Medi-Cal, making qualification much easier.
• Live in their own home (not a nursing home or licensed care facility).
• Be unable to safely live at home without assistance.

The applicant does not need to have a specific medical diagnosis. The need for help with daily activities is the determining factor.

How Many Hours Will IHSS Authorize?

A county social worker conducts an in-home assessment, scoring each task based on the time it takes and how often it is needed. The maximum is 283 hours per month (about 65 hours per week) for severely impaired recipients. The average IHSS recipient is authorized for around 100 to 120 hours per month. Authorized hours can be combined into the schedule that works best for the family.

Choosing and Hiring a Caregiver

One of the unique features of IHSS is that the recipient chooses their own caregiver. The caregiver can be:

• An adult child, sibling, or other relative (parents of adult children with disabilities qualify; spouses and parents of minor children generally do not).
• A friend or neighbor.
• A caregiver hired through the county Public Authority registry.

The caregiver must complete a county enrollment process, including fingerprinting, a background check, orientation, and a TB test. Once enrolled, the caregiver is paid by the state via direct deposit, typically twice a month.

2025 Caregiver Pay Rates

IHSS wages are negotiated county by county and currently range from $16.50 to $20.00 per hour, plus paid sick leave and (in some counties) health benefits. Caregivers do not pay payroll taxes if they live with the recipient and meet certain IRS criteria, which often makes IHSS earnings tax-free for live-in family caregivers.

How to Apply

1. Contact your county IHSS office to request an application.
2. Confirm or apply for Medi-Cal eligibility through your county human services agency.
3. Complete the IHSS application (form SOC 295) and return it to the county.
4. A county social worker will schedule an in-home assessment, usually within 30 to 45 days.
5. The social worker authorizes a Notice of Action listing your monthly hours and the start date of services.
6. Choose a caregiver, complete the provider enrollment, and begin services.

Common Reasons Hours Get Cut

• The social worker did not see the recipient struggle with a task during the visit.
• The recipient downplayed the difficulty of certain tasks during the assessment.
• Documentation from the physician was vague or missing.

If you disagree with the authorized hours, you have the right to request a State Hearing within 90 days. Many families successfully restore or increase hours through the appeal process. Bring detailed daily logs and physician letters to the hearing.
