Why 24-hour care costs three times what live-in care costs
Awake shift care is billed by the hour, so every one of the 720 hours in a month is paid at the full rate. Live-in care is billed as a flat daily rate because the caregiver sleeps at the home and takes an uninterrupted sleep break, so the agency is not paying for awake coverage overnight.
That is the whole difference, and it is worth about $15,000 a month. The decision is a clinical one, not a budget one: if the person wakes repeatedly, wanders, needs help to the bathroom at night, or is a serious fall risk after dark, live-in staffing is not safe and shift care is the right answer.
