What Is a Patient Transfer?
A transfer is the process of moving a person from one surface to another - such as bed to wheelchair, wheelchair to toilet, or chair to standing. Unsafe transfers are one of the most common causes of falls and caregiver injuries in home care settings.
Why Use a Checklist?
Even experienced caregivers can rush or skip steps under time pressure. A checklist ensures every safety measure is followed consistently, reducing the risk of falls, skin tears, and back injuries for both the caregiver and the person being transferred.
When to Use a Transfer Device
If the person cannot bear weight, is significantly heavier than the caregiver, or the caregiver has back problems, a mechanical lift (Hoyer lift), transfer belt, or slide board should be used. Never attempt a manual transfer beyond your physical ability.
