SCAN Health Plan
SCAN is a not-for-profit carrier based in Long Beach that has been writing Medicare Advantage policies in California for more than four decades. The plan has built a reputation for leaning heavily into supplemental benefits aimed at helping seniors age in place.
On many of its California plans, SCAN includes an annual allowance of in-home support hours that can be used for personal care or caregiver respite. Members typically work with a SCAN care navigator who arranges the service through a contracted home care agency, so the family is not left to find a provider on their own. Anthem Blue Cross
Anthem is the largest commercial insurer in California and offers a wide range of Medicare Advantage products, from zero-premium HMO plans to richer PPO options. Because the plan menu is broad, supplemental home care benefits vary significantly from one Anthem plan to the next. Two Anthem members in the same county can have very different coverage.
Most Anthem Medicare Advantage plans in California offer some form of post-discharge support, including a defined number of meals and a transportation allowance for medical appointments. Select plans add an in-home personal care benefit, usually framed as "essential extras" or under a chronic condition special needs (C-SNP) plan for members with diabetes, heart failure, or COPD. Anthem also offers a separate Dual Eligible Special Needs Plan (D-SNP) for members who qualify for both Medicare and Medi-Cal, and these D-SNPs often have the most generous home care hours because they coordinate with California's IHSS program.
What to verify during enrollment: whether your parent qualifies for a C-SNP or D-SNP (the home care benefits are noticeably better on those), how many personal care hours are included, and whether the plan reimburses a family member or only a licensed agency caregiver.
Alignment Health Plan
Alignment is a newer, technology-focused California carrier headquartered in Orange. It has grown quickly by marketing itself around concierge-style care management and high-value supplemental benefits.
Many Alignment plans bundle an in-home support benefit branded as "Companion Care" or similar, which pairs members with a non-medical aide for a set number of hours per week or per year. Alignment also tends to include generous allowances for over-the-counter items, groceries (often loaded onto a debit-style card), and transportation. A dedicated concierge team helps schedule the home care service, which removes a layer of friction for families who have never hired an aide before.
What to verify during enrollment: how Companion Care hours are scheduled (some plans require weekly visits, others allow flexible banking), whether the benefit resets each calendar year, and whether the plan covers your parent's specific specialists.

Do Medicare Advantage Plans Cover Home Health Care?
Yes, but the words matter. Every Medicare Advantage plan must cover the same "home health care" that Original Medicare covers, which means short-term skilled nursing, physical therapy, occupational therapy, and a home health aide while a person is actively recovering and is considered homebound. That benefit is not the same as ongoing custodial care. It runs for weeks, not years, and it ends when the patient stops needing skilled services.
The supplemental in-home support hours described above are different. Those are the non-medical, day-to-day help that families usually mean when they say "home care." They are optional benefits the plan chooses to offer, and they vary by carrier, county, and plan year. When you call a plan, be specific. Ask about "in-home support services" or "personal care hours," not just "home care," to make sure you and the agent are talking about the same thing.

Even the most generous Medicare Advantage supplemental benefit usually tops out somewhere between a few dozen and a few hundred hours per year.
