Why families need an mg to mL converter

The formula, step by step
The whole conversion rests on one relationship: volume (mL) = dose (mg) ÷ concentration (mg/mL). Reversed, it is dose (mg) = volume (mL) × concentration (mg/mL). Nothing else is involved, which is exactly why the errors that do happen tend to come from the inputs rather than the math.
Step one is to read the concentration correctly. Many labels state strength per 5 mL rather than per 1 mL, because 5 mL is a standard teaspoon-sized dose. A bottle marked 250 mg/5 mL is a 50 mg/mL liquid. If you type 250 into the concentration field instead of 50, the calculator will tell you to give one fifth of the intended dose. Reduce the label figure to a per-milliliter number first, every time.
Step two is to confirm the ordered dose. Use the prescriber's written order or the pharmacy directions, not memory from the last bottle. Weight-based pediatric and geriatric doses change as body weight changes, and the strength of a compounded suspension can change between pharmacies.
Step three is the sanity check. A single oral dose that comes out above roughly 20 mL, or below 0.2 mL, is worth a second look. Very large volumes usually mean the concentration was entered per 5 mL by mistake; very small volumes usually mean the opposite. Both deserve a call to the pharmacist before anyone measures anything.

Notice that the same 500 mg order produces 2 mL at one strength and 20 mL at another. That tenfold spread is the entire reason concentration has to be verified on the bottle in your hand rather than assumed.

Two reading habits keep this chart safe. First, convert a per-5-mL label into a per-mL figure before you pick a column: 250 mg/5 mL is the 50 mg/mL column, not the 250 mg/mL one. A 5 mL teaspoon is the row most families reach for, and it shows the point of the whole exercise: the same spoonful is 25 mg of one suspension and 500 mg of another. The volume never tells you the dose on its own.
Measuring the dose accurately
Once you have the volume, the measuring device matters as much as the math. Oral syringes are the most accurate option for anything under 10 mL and are the only sensible choice for doses that include a decimal, such as 2.5 mL or 0.8 mL. Dosing cups are acceptable for round volumes of 5 mL or more, but reading them at eye level on a flat surface is essential because a tilted cup can overdeliver by 20 percent or more.
Draw slowly to avoid air bubbles, then hold the syringe upright and tap it before adjusting the plunger. For someone with swallowing difficulty, give the liquid in small amounts along the inside of the cheek rather than straight to the back of the throat. Never mix a full dose into a large drink or bowl of food, because anything left behind is medication that was not delivered.
Shake suspensions for a full ten seconds before every dose. Suspensions separate on the shelf, and drawing from an unshaken bottle means the first doses are weak and the last ones are concentrated. Refrigerated antibiotics in particular settle firmly overnight.
Common dosing errors and how to avoid them
- Mistaking per-5-mL strength for per-mL strength. Reduce the label figure before entering it here.
- Reusing an old volume after a refill. Pharmacies substitute strengths routinely. Re-run the conversion on every new bottle.

Liquid formulations show up often in home care because they are easier to swallow than tablets and can be given through a feeding tube. They also introduce risk. Kidney and liver function decline with age, so an older adult clears many drugs more slowly and a dosing error that a younger person would shrug off can cause sedation, a fall, or a hospital visit. Practical safeguards help more than vigilance alone: keep one current medication list in the house, write the calculated volume directly on a label taped to the bottle, store the matching syringe with that bottle, and bring every container to each appointment.
